Heart Failure Management – For Patients and Caregivers

This course will help you learn more about the heart failure disease process, identify your heart failure needs and various treatment options. More importantly, you will develop an understanding of the team based care for this condition and the roles of your team members, including family and friends. Finally, you will have a chance to learn about patient experiences while living with heart failure. There is also a cooking demonstration including as part of this conference.
