Note Template – Preoperative Risk Stratification

  • The patient is at {low/moderate/high} risk of adverse cardiovascular outcome with non-cardiac surgery.
  • Standard surgical and anesthetic technique should be used.
  • Additional cardiac testing {is/is not} indicated.
  • Antiplatelet therapy with aspirin {can/cannot} be discontinued prior to the procedure.
  • Anticoagulation {can/cannot} be discontinued.  Recommend bridging with {UF/LMW} heparin to {goal INR}.
